ICAAC 2013 Top 10 papers in Mycology

今度は真菌症に特化したLiterature reviewです。例によってところどころ端折ります。また全部自分で読んでいるわけではないので内容の詳細についてはあしからず。

1. 診断
・前の記事でも紹介した骨髄移植を受ける患者さんに対して従来通りの病理と培養でアスペルギルスの診断をつける作戦と、PCRとガラクトマンナンを使って抗真菌薬の投与を決める作戦とを比較した研究です。

2.疫学とリスクファクター
・竜巻後の皮膚真菌症。

自然災害に伴う外傷に合併した真菌感染症ですね。13例が同定され、5例が死亡。全例がApophysomyces trapeziformisという真菌だったようです。戦闘による外傷に合併した糸状菌感染症もちらっと紹介されていました。

3.侵襲性真菌感染症に対する治療

4.汚染されたステロイド剤での真菌性髄膜炎のアウトブレイク

Exserohilumという真菌のアウトブレイク。米国ではだいぶ話題になりました。

5.耐性
Candida glabrataにキャンディン系の耐性が増えているという話。重複。

7.慢性、アレルギー性のアスペルギルス症に対する治療
慢性空洞性アスペルギルス症に対するItraconazole投与。RCTがされています。

慢性の肺アスペルギルス症は非常に難治な病態ですが、ここではITCZがSupportive careのみよりもアウトカムがよかったようです。CNPAにも応用できるのか、などなど興味がありますね。
