ICAAC 2013 Literature Review Session その1

久しぶりに機会をいただいてデンバーで行われたICAAC (Interscience Conference on Antimicrobial Agents and Chemotherapy) に参加してきました。

恒例のLiterature reviewのセッションを抜粋して紹介です。 抜粋というか自分が興味をもったものを紹介しているだけですね。ハンドアウトはこちら（リンク先PDF）で入手できるようです。いつまでできるかわかりませんが。

なお自分ですべての論文を読んで紹介しているわけではありませんのであしからず。

トップバッターは世界を震撼させた中東呼吸器症候群ウイルスの話題でした。どうでもいいことかもしれませんが演者たちはこのウイルスの略称であるMERS-CoVのことを『マースコヴィー』とか『メルスコヴィー』みたいな感じで呼んでいました。

続いては中国で出現した新型インフルエンザH7N9についてでした。

新しいインテグラーゼ阻害薬のドルテグラビルの論文が3つ紹介されていました。インテグラーゼ阻害薬も本格的にQDの時代に突入ですね。米国ではTivicayという商品名で既に発売されているようです。Tivicayはなんと発音するのかよくわかりません。（P. Sax先生もよくわかんないみたいです。そんなP. Sax先生を会場でちらりお見かけしました）

ARTの投与は早いほどCD4の回復にもよい影響があるのだという話。

Primary infectionの時点でARTを始めると長期的にもHIVのリザーバーがヘリ、T細胞の機能が保たれる。

ARTでコミュニティーのHIV伝播が減らせるようだ、という話。

HIVの世界でもどんどん薬剤を使う方向に話が進んでいますね。

続いてCryptococcusの話。

以前からCryptococcus髄膜炎の時にアムホテリシンBにフルシトシン（5-FC）を併用するのがスタンダードされていましたが、アムホテリシンB単剤と比較して死亡率の低下効果があるのか不明でした。そこでAMPH-B vs. AMPH-B + 5-FC vs. AMPH-B + FLCZというRCTが行われてAMPH-B+5-FC群が6ヶ月の時点での生存に有意によかったという結果でした。ちなみにこの研究はベトナムで行われて、患者は全員HIVでかなりCD4の低い人達ではあります。

Cryptococcus gattiというまだ日本では出てきてはいないタイプのCryptococcusに対する治療のまとめ。

PCRでルーチンに検査するようにしたら院内発症のレジオネラ肺炎が結構いるようですよ、というNZからの報告。
